Claim Denied? How to Appeal Your Marketplace Plan’s Decision — and Win
ACA plans must offer internal appeals and independent external review. A step-by-step guide to fighting a denied claim.
A denial letter is not the end of the conversation — it’s an opening position. The ACA guarantees a structured appeals process, and a striking share of appealed denials get overturned. Most people simply never appeal.
The two-stage process
Start with an internal appeal: request the denial reason in writing, get a letter of medical necessity from your doctor, and file within the deadline on your notice. If the plan upholds its denial, you’re entitled to external review by an independent third party whose decision binds the insurer. Keep every document and date. Persistence is genuinely the deciding variable — and your agent should be in the fight with you.
What every ACA plan must cover
Ten categories of care are guaranteed in every Marketplace plan: outpatient services, emergency care, hospitalization, maternity and newborn care, mental health and substance-use treatment, prescription drugs, rehabilitative services and devices, lab work, preventive and wellness services, and pediatric care including dental and vision for children.
That floor is what separates real insurance from the lookalike products sold in ads. Any plan missing one of the ten is not ACA-compliant coverage, whatever the marketing says — and the gap will be exactly where the expensive claim lands.
Reading a plan’s network before you buy
Network type is the first filter: HMOs typically require a primary-care gatekeeper and referrals, EPOs drop the referrals but keep the closed network, PPOs open the gate to out-of-network care at a price. In most Texas counties, Marketplace shelves are stocked mainly with HMOs and EPOs, so in-network discipline matters.
The practical test is simple: your doctors, your hospital, your medications, checked against the specific plan — not the carrier generally, since one insurer runs many networks. It’s tedious. It’s also the single highest-value fifteen minutes in insurance shopping.
Anchor your plan choice to your providers
The most reliable way to be happy with a health plan is to shop backwards from the care you already trust: list your primary doctor, your specialists, your preferred hospital, and your pharmacy’s convenience, then filter plans to the ones that contain them.
Verify in the plan’s own directory and confirm by phone with each office — “do you accept [specific plan name] for next year?” Provider participation changes annually, and offices know their contracts better than any website. Ten minutes of calls prevents a year of regret.
The free care you’re already paying for
Every ACA plan covers a substantial list of preventive services at zero cost-sharing when delivered in-network: annual wellness visits, immunizations including flu shots, blood pressure, cholesterol and diabetes screenings, major cancer screenings, well-child visits, and more. No deductible, no copay.
This is the benefit that pays you to use it — catching conditions early, when treatment is simpler and cheaper. If your household hasn’t had its annual visits this year, they’re already covered. Book them.

Final thoughts
Health coverage decisions feel overwhelming mostly because the information arrives all at once. Take it one step at a time, use the free help that exists, and remember: enrolling through a licensed agent costs exactly the same as going it alone.
